FOSTER AGREEMENT
I understand and agree that the Foster Dog is the sole property of BLR and will surrender the Foster Dog immediately upon request. I acknowledge that I do not have the right or authority to keep or place the Foster Dog in any home or with any other individual, or boarding without the prior written consent of BLR.
I certify that no person residing in the home the Foster Dog will be cared for has ever been charged or convicted of animal cruelty, neglect, or abandonment. I agree to notify BLR immediately if any person residing in the home is charged in the future.
I agree to provide the Foster Dog with adequate food, water, shelter, affection, socialization, exercise, and to strive to increase the adoptability of the Foster Dog. I agree to be patient and tolerant and work with the Foster Dog on good house manners and basic obedience, I will address any issues which will help the Foster Dog more adoptable. I agree to never employ harsh training methods or allow another person to physically or emotionally abuse the Foster Dog.
I agree to notify BLR immediately if the Foster Dog is lost, stolen, or injured. I also agree to never take the Foster Dog to any dog park.
I agree to provide BLR continuous updates on the Foster Dog pertaining to the health, temperament, personality, and training. I agree to also provide continuous pictures and videos of the Foster Dog.
I understand that pictures and videos are used to promote the Foster Dog on social media and other websites. I give my consent for BLR to use any picture and/or video for that purpose that may include myself and/or my minor children.
I understand that BLR reserves the right to check on the welfare of the Foster Dog at any time, which could include a home visit, and agree to allow BLR access to check on the Foster Dog.
I understand that BLR provides for the medical care of the Foster Dog through designated vets. I agree to take the Foster Dog to vet appointments or provide BLR advance notice if unable to so that alternate arrangements can be made. I agree to administer any medication prescribed to the Foster Dog according to instructions given by the vet or BLR. I understand and agree that expenditures other than life threatening emergency care must be pre-approved by BLR or I will be responsible.
I understand that BLR may euthanize the Foster Dog at any time for medical and/or behavioral reasons, including aggression or quality of life concerns. I understand that these decisions are not made lightly and will respect those decisions.
I understand that foster dogs may be or become sick while in my care and I agree it is my responsibility to ensure my personal pets are current on vaccinations. I understand and agree that BLR is not responsible for providing care to my personal pets
In the event that I am ever unable to continue caring for the Foster Dog, I agree to provide BLR at least a seven-day-notice to allow time to locate another foster home for the Foster Dog. I also agree to promptly return all BLR supplies, equipment, records, moneys, and other items I may have in good and clean condition if I will no longer be providing care in the future.
I understand and agree that I cannot adopt my first Foster Dog. I also understand that if I want to adopt a Foster Dog after the first one I must make that known within the first five days of having the Foster Dog. After the five days, the Foster Dog is available to applicants as well as myself. I understand that once the Foster Dog has been offered to someone I cannot stop that adoption. I also understand that if I state that I want to adopt my Foster Dog, I must be prepared to complete the adoption at that time.
I understand that as a Volunteer/Foster, I may gain access to information about other BLR volunteers, fosters, applicants, adopters, and/or shelters that is confidential. I agree to maintain confidentiality and to refuse disclosure of any information that is either private or personal.
I agree to assume the risks implicit in working with animals that have been abandoned, neglected, mistreated, or who may suffer from an illness or disease. I certify that my participation in BLR volunteer/foster program is entirely volunteer. I understand that BLR may have limited knowledge of the Foster Dogs behavioral and/or medical background. I also understand and agree that BLR makes no warranties or representation regarding the Foster Dogs health, behavior, temperament, or age.
On behalf of myself and my heirs, personal representatives and assigns, I hereby discharge, idemnify, and hold harmless BLR and its directors, officers, employees, and agents from all claims, lawsuits, injuries, damages, losses, costs, or expenses whatsoever which I might suffer or sustain in connection with the volunteer/foster activities for BLR.
I affirm with my signature below, I have read and understand this agreement in its entirety and I agree to abide by the terms of this agreement. I understand and agree that this form of electronic signature has the same legal force and effect as a manual signature.
