Although our adoptable animals have had excellent care while in our custody, it is the nature of rescue work that not everything about the history or status of an animal can be known. For this reason, NO GUARANTEE CAN BE MADE REGARDING THE CURRENT, FUTURE, OR PAST HEALTH OF THE ANIMAL. By signing below, you recognize that this previously stray animal may expose the other animals and people in your household to disease. Household members that are young, have compromised immune status, or are pregnant may be at a higher risk for contracting zoonotic diseases from any animal, including the adoptable pets we showcase and adopt out. You understand that handling any stray animal may place you or others with you at risk for being scratched or bitten, as well as potentially exposing you to parasites and bacterial, viral, or fungal diseases. You recognize and understand the risks inherent in handling any animal AND AGREE TO INDEMNIFY AND HOLD HARMLESS CAT'S BEST FRIEND RESCUE, ALL STAFF, VOLUNTEERS, AGENTS, EVENT HOSTS AND ASSOCIATED BUSINESSESS OR ORGANIZATIONS FOR ANY INJURY, DAMAGES, CONSEQUENCES OR DISEASE YOU, YOUR FAMILY, OR OTHERS ACCOMPANYING YOU MAY SUSTAIN OR BE EXPOSED TO. By signing below, you certify that you are 18 years or older and you have read and understood this document. IN SIGNING THIS DOCUMENT, YOU HEREBY FOREVER WAIVE ANY CLAIM TO A REFUND. Terms and Conditions

BY COMPLETING AND SIGNING THIS APPLICATION, YOU AGREE TO: • Allow me to contact your landlord, homeowner’s association, and Veterinarian to complete references.

• Required home visit and/or share photos of your home. Video and/or pictures should be sent to contact@catsbestfriend.org

• Contact me to arrange return transport in the event you can no longer keep this cat; I will not abandon, re-home or sell cat after adoption is completed.

• The cat/s will be examined by your veterinarian within 7-14 days of adoption and a copy of the evaluation and treatment plan sent to contact@catsbestfriend.org

• Continue and provide adequate medical to your cat/s as needed.

• You will not declaw cat/s

• You will provide a safe environment within your home.

• Your cat is being adopted into your family and will be indoors only.

• Your cat will never be euthanized unless in the case of terminal illness, injury, or old age to prevent pain and suffering; euthanasia will be completed by a licensed veterinarian.

COST: The adoption fee for your cat/kitten will be $250 per pet.

This fee will include:

Spay/Neuter

FVRCP Vaccinations – 2 minimum, the 3rd recommended vaccination may need to be provided by your vet at their first appointment.

Rabies Vaccination –Your cat or kitten has received a 1-year rabies vaccination, if the vet deems it old enough to receive prior to transport. If it has not received a rabies vaccination, you will be responsible for having that done and sending a copy to contact@catsbestfriend.org.

Deworming – Cats/kittens are dewormed monthly.

Flea Treatment – your cat or kitten has received a topical flea treatment within one week of transport.

TRANSPORT: If your cat or kitten requires transport greater than 4 hours, the cost for transport will be in addition to the adoption fee and will be paid directly to the transporter at pick up. If a service is required and I cannot make the transport myself or you cannot pick up the cat/kitten. The cost of transport will be determined by the company selected. I am happy to help you find transportation for your new family members if needed.
